There are three basic steps to enrollment:

Step 1. Use the CostConnect medical estimator to explore options for the medical plan based on your projected actual usage. CostConnect will also help you make a better estimate for your Health Care FSA, should you choose to participate. Click on the following link to go to www.icicostconnect.com.

Step 2. After you have read the Enrollment Guide and understand your choices, go to the eBen link at the top of the screen and follow the enrollment instructions there.

Step 3. Complete the Health Risk Assessment (HRA), if you have not already done so. To complete, go to www.netbenefits.com. If you have not previously registered on that site, go to "New User Registeration". Call 800-971-6832 if you need further help.

Deadlines: Be sure to enroll by the deadline shown in your decision kit. If you don't enroll by the deadline, you will receive default coverage (Option D-BasicRx, employee only and no dental). Complete the Beneficiary Form included in your enrollment kit and return it in the envelope provided.
